Privacy notice for website inquiries and project requests.
This notice describes the information handled through this website and the third-party services intentionally used for project requests and scheduling.
Information you provide
Project requests may include your name, work email, organization, product stage, clinical or technology area, geography, project focus, objective, timeline, regulatory or market status and other information you choose to provide. Do not submit protected health information, patient records, passwords, payment-card information or confidential information you are not authorized to share.
Formspree
Website project requests are transmitted using Formspree. Formspree may process form contents and technical information needed to deliver the submission under its own terms and privacy practices.
Calendly
The website links to Calendly for scheduling an introductory call. When you follow that link, Calendly may process information and use cookies or similar technologies according to its own privacy practices.
Technical information
Hosting and network providers may receive technical request information such as IP address, browser and device information, requested URL, timestamps, and security or delivery logs to operate and protect the site.
How information is used
- Review and respond to project, advisor-network or business inquiries.
- Determine whether a proposed engagement fits the firm’s capabilities.
- Prepare or discuss a scope, proposal, statement of work or business relationship.
- Maintain business, security and compliance records.
- Protect the website and firm from misuse or security threats.
Sharing
USHSG does not sell website form information to advertisers. Information may be shared with service providers that support website or communication functions, with advisors or operating partners when necessary to evaluate an authorized engagement, or when disclosure is legally required.
Project confidentiality
A website submission is not a substitute for a signed nondisclosure agreement or project agreement. Sensitive business information should be exchanged only under appropriate confidentiality terms.
Retention
Business inquiries and related correspondence may be retained as reasonably necessary to respond, administer a relationship, document activity, resolve disputes, maintain security or satisfy legal and accounting requirements.
Security
Reasonable administrative and technical steps are used to reduce unauthorized access or misuse, but no internet transmission or storage system can be guaranteed completely secure. Website forms should not be used for highly sensitive or regulated information.
Tracking posture
The current site does not intentionally include an advertising pixel, behavioral-tracking package or non-essential analytics package. If that changes, the notice and any required consent controls should be updated with the deployment.
